POST /v1/physicians/{id}/heartbeat: the presence after the write, plus
the consult the physician is now on or being rung for — because going
ready (and every beat while ready) retries the match, and a match is the
thing the caller wants to hear about first.
This physician's ringing or in_progress consult after the match
attempt, or null when idle. in_progress → call /room for the grant;
ringing → keep polling the workspace until the patient confirms.
