Body of POST /v1/telehealth-consults/{id}/end — hang up. Idempotent on a
consult that already ended (returns it unchanged, next: null).
With a physician credential the caller must be the consult's practitioner
(409 not_assigned). With the tenant key alone this is an administrative
end of somebody else's call; the practitioner on the row is still put back
on the rota (or offline) exactly as if they had hung up themselves.
