turns the messaging thread into a live video consult. An empty object (or
omit it).
Requires a physician credential naming the CURRENT assignee (409 not_assigned) and an open thread (409 thread_not_active). The platform
opens a telehealth consult for the same patient, closes the thread with
closedReason: 'escalated', and sends the patient the escalated notice
with their join link through their usual channel — for your patients, onto
the transcript and the async_consult.message webhook, exactly as the
portal does it. The response names the new consult so your physician UI can
go ready for it.
