close it — a physician nobody is watching for must fall off the rota, or
patients are rung for a clinician who never answers. A heartbeat on a
physician who is not ready records lastSeenAt and changes nothing else
(it does not revive a lapsed ready; post presence again for that). While
ready, a beat also retries the match, so activeConsult may appear here.
